Hard to tell without context. Can you share what device it came from or any markings on the other side?

What components surround it? Are the traces off the pins short or long? Do any pins run (fairly) directly to a connector? What is that connector for? What does the board do? What is the board from?

The numbers are possibly proprietary, contact the equipment manufacturer to check, they might or might not share the info. The circuit board designation U12 might indicate an op amp, or maybe a dual op amp, just a guess though. Have you tried searching for a schematic for the equipment it is in?
